Have You Received a Re-Validation for Medicare?

Many office are receiving a re-validation request for Medicare. Groups/Organizations must revalidate one of two ways.
    1.  Complete and submit a paper CMS 855B Form NAS “Enrollment” page at                   https://www.noridianmedicare.com/partb/enroll/ contains a hyperlink that  will direct you to the “CMS Forms List” at  http://www.cms.gov/Medicare/CMS-Forms/CMS-Forms/CMS-Forms-List.html. Once on that page, enter the “Form #” in the “Filter On” box, select “Form  #,” and located under “Downloads,” select the “Form #” PDF. Older  versions of the application cannot be processed. In Section 1a of the CMS 855B, include all active PTANs the group or organization has under that EIN with Medicare. Include all valid practice locations in Section 4 (copy this section for each location). The appropriate NPI and PTAN for each location should be indicated in each Section 4. This form and the supporting documentation must be returned to our office within 60 days of our request or your Medicare billing privileges may be deactivated or payments will be withheld.  Requires the original signature and date of the Authorized Official in              section 15 (We recommend blue ink).  Applications process in an average of 60 days.

    2.  Complete an internet based application on the Internet based Provider Enrollment, Chain and Ownership System (PECOS) at https://pecos.cms.hhs.gov/pecos/login.do.
